Plan Information.If your plan is a PPO type plan with "out-of-network" benefits, our office accepts benefits that your insurance company provides. Depending on your particular plan you may have full coverage for some procedures and partial coverage for others.

Please understand that your insurance is a contract between you, your employer and the insurance company. We are not a party to that contract.

Our fees are considered to fall within the acceptable range by most insurance companies, and therefore are covered up to the maximum allowance determined by each carrier. However, not all services are covered by insurance benefits in all contracts. Some insurance companies arbitrarily select certain services they will not cover.

At all times there will be a deductible and personal out of pocket expenses incurred to you.

We must emphasize that as dental care providers, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date the services are rendered. Understand that, regardless of insurance status, you are ultimately responsible for the balance of your account.

Claims.Our office will file claims for you electronically, so in most cases you will not need to bring insurance forms with you. If you provide us with accurate information about your insurance plan we would be glad to generate an estimate that summarizes your total out-of-pocket costs for treatment that is required and has been agreed-upon.
